The VPAG rebate is negotiated on behalf of all four UK nations. The reduction of the VPAG rebate from 22.9% to 14.5% therefore applies to NI as does the commitment to increase spending on innovative medicines from 0.3% to 0.6% of GDP. Northern Ireland endorses NICE technology appraisals through a separate DoH NI process. The nine additional medicines approved under the higher NICE threshold since April will each require a NI endorsement decision and then a Service Notification to HSC Trusts. 1
The fast-track access pilots announced on 2 July are NHS England–specific. However, the pressure they create is relevant to NI, where the 2025 Medicines Access Review identified governance infrastructure as the bottleneck between endorsement and implementation. The DoH NI’s endorsement of NICE TAs routinely makes treatment available in NI on the same basis as England. As the pace of approvals increases, so does the governance burden on SPPG. 2
Blueteq comment: The 2025 Medicines Access Review has identified the absence of a specialist medicines software system as the primary governance gap — and recommended Blueteq. The combination of a busier pipeline and an unresolved governance deficit is a strong case for implementation.
